Physical Information:

• The Triennial Physical needs to be filled out every three years. This includes an exam by a physician and requires a physician's signature. 
• The Interim Physical needs to be filled out every year, except the year the triennial physical is filled out. This physical only requires parent and student signatures. 

The physical needs to be filled out if student participates in any school sponsored sport.
